يُعلن بموجب هذا أن شركة Clay Fleet Express ذ.م.م.، الواقعة في 12049 Almer Lane، Chester VA 23836، قد تقدمت بطلب للحصول على شهادة صلاحية تخولها نقل مستفيدي Medicaid بمقابل مادي داخل Virginia بصفتها ناقلاً طبياً غير طارئ.  

أي شخص يرغب في الاعتراض على هذا الطلب يجب أن يرسل بياناً مكتوباً يوضح (1) بياناً دقيقاً باعتراضات الطرف على الموافقة على الطلب، و(2) بياناً كاملاً وواضحاً وقائعياً يكون الشخص مستعداً لتقديمه بأدلة معتبرة، و(3) رقم القضية المخصص للطلب، و(4) شهادة تفيد بأن نسخة من الاعتراض قد أُرسلت إلى مقدم الطلب. رقم القضية المُخصص لهذا الطلب هو MC2600702ST. The written statement must be sent to: DMV, Motor Carrier Services – Case Management, P. O. Box 27412, Richmond, VA, 23269-0001 and postmarked on or before October 19, 2026.  The only grounds for protests are fitness allegations.
    
Any protest filed with competent evidence will be carefully considered by DMV.  However, DMV will have full discretion as to whether a hearing is warranted based on the merits of the protest filed.
